Please read all information below before you begin the on-line registration process.

COMPLETING THE APPLICATION

You will be able to apply only to the waiting lists that are open. If you are unable to check the box for one of the waiting lists, it is because the waiting list is not open at this time.

You may complete an on-line application form at any time. Upon completion you will receive a Pre-Application receipt containing a confirmation number acknowledging successful transmission of your information. However, by submitting this application you are not guaranteed housing assistance.

Before you begin the pre-application process, please have available the social security number, name, date of birth and income information for each family member. Enter all information carefully and check to make sure it is correct before you complete the application.

As you complete the application, look for the following symbol: * This means the information is required.

Important: To move from one step of the application to another, you must use only the buttons at the bottom of the application. (The Begin button is below. Other steps have Next, Back, Confirm, and/or Submit buttons at the bottom that allow you to go forward and back within the application.) Using ANY OTHER option, including right-clicking with your mouse and selecting Back or Forward, will cause an error and may force you to begin a new application.

MAINTAINING YOUR STATUS ON THE WAITING LIST

It is your responsibility to assure that all information remains current. When your on-line application comes to the top of the waiting list, you will be notified by electronic (e-mail) or regular mail. If we are unable to contact you because the e-mail address is no longer valid or a letter is returned as undeliverable, your name will be removed from the waiting list. If your application is removed from the waiting list, you will not be able to re-apply until the wait list re-opens.

ELIGIBILITY

Eligibility requirements include the following:
  1. All household members are elderly or disabled; or;
  2. One adult member of the household is employed or has a confirmed offer of employment for 20 hours or more per week at the State minimum wage; and,
  3. One adult member must demonstrate past employment for a minimum of 12 months immediately preceding application; and,
  4. All other household members’ ages 18 through 54 must be employed 20 hours or more per week or engaged in an education or training program.
